Applying for child support services requires that you provide personal information about the other parent and about yourself. For security purposes, we will ask first-time online applicants to complete a short registration process. Returning applicants may use the previously established user e-mail address and password.

For your convenience and to expedite the receipt of your application for child support services, we recommend that you submit your application online; however, customers who prefer to download and mail an application may do so by accessing our forms section and selecting, download form.
