hgThe Commuter Assistance Program office distributes many brochures, schedules, maps, and applications. Most of the materials are available at the self-service Commuter Information Display Racks located at over 30 County of Orange worksites. The following materials are available on-line or at the Commuter Information Display Rack Location near you:

• Commuter Assistance Program Guidelines - This document summarizes the program and eligibility requirements.
• All-in-One Application - Use this form to register for the "Carpool Classifieds", apply for the "Try the Train on Us" or Try the Bus on Us" complimentary tickets, and register for the "Commuter Club".
• Voluntary Payroll Deduction Authorization Form - Use this form to purchase bus, train or vanpool fares with pre-tax dollars through the payroll deduction process.
• Guaranteed Ride Home Authorization Form - Use this form to request an emergency ride home on a day that you carpool, vanpool, ride the bus or train.
• Carpool Connection - This document provides basic information about carpooling, finding a match and what to discuss.
• Refer-A-Friend - Use this form to refer co-workers to the Commuter Assistance Program and the Commuter Club Rewards Program.
• Advantage/Option Rideshare - Use this form to apply for an incentive to start ridesharing from the Inland Empire.
• Rideshare Plus - Use this form to apply for rewards for ridesharing more than three months from the Inland Empire.
