Child Abuse Reporting Follow-Up Forms

Mandated reporters are required by Penal Code Section 11166 to make an initial child abuse report via telephone with a follow-up via written or electronic means within 36 hours.

There are two options for mandated reporters to submit their required written follow-up. To simplify the process, an online follow up application has been developed for your convenience. Mandated Reporter Online Follow-Up Application

**Please note, in order to access this application you first need to make your child abuse to the Child Abuse Registry at (714) 940-1000. The Registry worker will provide information with how to complete your mandated follow-up using this application.
